I hereby request that the Brevard Federation of Teachers (BFT) terminate my union membership and my ACH (eDues)/Personal Check deductons for professional dues, I understand that 5 business day signed written or electronic notice will be given to BFT to terminate my membership. Furthermore, I understand that if an ACH eDues deduction is scheduled within the five days of cancellation notice, the dues deduction will occur as scheduled. I understand that if my dues were paid by check, I will only be reimbursed for the remaining balance of dues. I understand that such request will void my professional membership in the Brevard Federaton of Teachers, Florida Education Association, American Federation of Teachers and the National Education Assocation. I understand that I will forfeit all the associated rights, benefits, and privileges as a BFT member. I further understand that no portion of dues previoulsy deducted will be returned to me.
